Frogspawn and Hammer Coral Issue

Recently I have been having some issues with some hammer and frogspawn heads melting slowly or just falling off. It takes a few days for it to happen. I checked the water parameters and they are looking good.

PH 8.2
ALK 10
Calcium 380
Amonium, Nitrite, Nitrate all 0
Phosphate-tested with phosphate checker meter .04

All other corals look good. I dipped the frogspawn in coral RX about a week ago and it seems to be better but the hammer just started doing the same. I dipped the hammer tonight.

Has anyone seen this before? Also it looks like the mouth of some of the polyps is gaping similar to anemone. Here is a picture.

There has been some on certain polyps.

in that case, I would look into brown jelly disease
your descriptions seems to match the signs

adding any Iodide. I found this to be key for the Euphyllias
